How can your doctor tell if your babys head is engaged if they dont feel your belly?

Im 36 weeks pregnant and baby's head was engaged 2 weeks ago but had my hospital appointment today and the lady didn't do anything but wrote in my book NE. arnt they meant to feel your tummy to see how far the head is down. I no the head engages and disengages but as if she can tell by looking?
